If you have a request for a dermatology consultation that you want to send to the clinic, please fill out the form below. 

Since all the requests of our clinic are processed via the CRDS and therefore the consultations transmitted via this form will automatically be sent to the CRDS, the requesting physician must fill the official CRDS form. If the requesting physician used the wrong form, the request will be sent back to him so he can provide the clinical information needed for the proper handling of your request.

We are part of CIUSSS Estrie-CHUS. If you live in another region, your request will be redirected to the appropriate CRDS (Please refer to the map above in case of doubt. St-Hyacinthe and the South Shore of Montreal are NOT part of the CIUSSS Estrie-CHUS.) Here is the link to find dermatologists in your area.
